Formidable Forms Surveys Addon v1.1.4

Formidable Forms Surveys Addon is an extension for Formidable Forms that turns standard forms into structured surveys with built in reporting. It solves the problem of collecting answers with no clear way to interpret them inside WordPress. Rather than exporting entries to Sheets and building charts by hand, you get survey fields and automatic result graphs in the dashboard, which saves time and keeps all response data in your own database.

You can use it to build customer satisfaction surveys, post project feedback forms, student polls for online courses, and research questionnaires for non profits. Agencies can create branded feedback systems for clients that live directly on the client site. Since it runs inside Formidable Forms, you keep conditional logic, multipage layouts, and embed options through shortcode, block, or widget area. Day to day, you can review charts, check individual responses, and export to CSV when deeper analysis is needed.
