Formidable Forms Surveys Addon v1.1.6

Formidable Forms Surveys Addon is an extension for Formidable Forms that adds survey focused fields and built in reporting to your WordPress forms. It solves the problem of collecting feedback with plain contact forms that lack structured answers and visual summaries. Instead of exporting CSV files to Google Sheets or paying for an external survey tool, webmasters can collect ratings and poll answers and review summarized charts directly inside the dashboard.

You can build student satisfaction surveys for online courses, post project feedback forms for small business clients, reader polls for blogs, and voting pages for communities or contests. The addon works with Formidable Forms core features like conditional logic, email notifications, and entry management, plus shortcode and block output for front end results. Day to day, you can trigger follow up questions on low scores, send admin alerts on detailed feedback, filter responses by date or user, and export clean CSV data for further review.
