Formidable Forms Quiz Maker Addon v3.1.7

This is an official extension for Formidable Forms that turns any form into a graded quiz. Instead of only collecting entries, you can mark correct answers, assign points, and show a total score after submission. It fills the gap between a basic contact form and a full LMS. You keep the drag and drop builder, conditional logic, and entry management while adding scoring without a separate quiz system or custom calculations.

You can build weekly classroom tests, lead qualification checks, membership certification steps, and employee training assessments. Teachers can mix scored radio, checkbox, dropdown, and scale questions with unscored name, email, and feedback fields. Small businesses can trigger different emails, CRM tags, or download links based on pass or fail logic. Since every attempt is stored as a normal Formidable Forms entry, you can filter by score, export to CSV, and review results from the WordPress dashboard daily.

Formidable Forms Quiz Maker Addon Key Features

• Quiz Enabled Fields: Turn radio buttons, checkboxes, dropdowns, and scale fields into scored questions while keeping name and email fields unscored.
• Correct Answer Marking: Select the correct choice directly in field settings and let the addon handle grading automatically.
• Weighted Scoring Per Question: Assign different point values so important questions carry more weight in the final grade.
• Automatic Score Calculation: Calculate total score and percentage on submission without custom formulas or extra code.
• Score Based Confirmations: Show different messages or redirect pages based on score ranges, such as congratulations above a set percentage.
• Answer Visibility Control: Show only the score or display a full breakdown with correct answers highlighted after submission.
• Pass and Fail Threshold: Define a passing score to separate qualified submissions from those that need review.
• Conditional Follow Up Actions: Trigger specific emails, CRM tags, or download links only when a taker passes using conditional logic.
• Entry Based Result Storage: Store each attempt as a standard Formidable Forms entry, filter by score, export to CSV, and view results in the dashboard.

Download Latest GPL Version of Formidable Forms Quiz Maker Addon

Use the GPL files to test grading behavior on a staging or local site before grading real students or leads. Check weighted points, percentage calculations, pass thresholds, confirmation messages, and answer display settings across different field types. Sandbox testing prevents broken scores or wrong redirects on a live site. Once your quiz flow is stable, purchase an official license for production use to receive updates and support.
